Don, I don't know why anyone was rude -- perhaps they were just being blunt.  Your report is nothing special and I have done up forms for many of Argent's subcontracting abstractors for free because I'm used to creating form-fill.  That's all that you're offering and anyone can do up a form with Adobe or CutePDF without cost while keeping control over what the categories are.

My own software uses something a bit like what you show, but that's what we use to process/track/invoice/store orders -- it's not just a form fill report.  My own forms have all of my company information and disclaimers already in place and you just tab between areas of the report.  Easy peasy/freezy breezy.  I supply them with every outgoing order, and like I said, I have helped many abstractors do up similar forms themselves -- without cost.
