I believe that it varies.  TRID places liability on the lender if the Closing Disclosure is wrong, so I think it is common for lenders to prepare the form.  However, lenders can delegate that function to a title company -- the lender still remains liable.  Either way, the lender and the title company have to have a good line of communication.
