I recently checked out Legal Zoom when I was forming an LLC for my real estate investment business.  Years back, when I incorporated Jireh, I used American Incorporators, Ltd., so I decided to do some comparison shopping.  I found that AI was more competitively priced, so you may want to check them out as well.

As far as what business structure you should use, I'm no lawyer, and this should not be construed as legal advice, but I understand that an LLC affords a little more protection, while a Sub-S Corporation has certain tax advantages.  Most of these online incorporation services will do a name search as part of the package.
