Good points brought up, George.  There have been numerous cases, as you describe above, where the doc that clarifies the search results have been accepted and paid for, where without it, the report would be queried by the client.  We're in agreement on this.  Some better examples of copies not requested would be:

We asked only for pertinent pages of a mortgage (first, last and page with legal description) and instead were sent the entire mortgage.

We asked for a mini search with only voluntary liens and instead were sent a report containing involuntary liens as well.

Sending a covenant  (which can be 50+ pages) with a report that was not requested or needed.

There are other examples but I think I clarified my point.  In the end, there is nothing that substitutes for good communication that leads to understanding between abstractor and client.  These things can and usually get worked out.  No one is perfect, so I hope for best efforts and cooperation on both sides so we can get the work done and earn a living at the same time.
