I had the same misunderstanding about the releases that were "almost right" then inquired with a couple of the real-estate attorneys in the record room( back when there were lots of people actually in the record rooms) and was advised of my error in doing what I was doing- the same as you do- report it as a defective release and then get a certified copy to make the correction- it used to cost me lots of money and time to do this- go in, get the cert copy, have it fixed then go record it- too much when all I needed to do was recognize that the complete document had to be taken in to account as to it's validity- and have had NO problems since making that adjustment- I do keep copies for my files of everything in case I get a call down the pike regarding anything - assuming that the next search is going to come up with the same objection- unless it is done by the famed "outsourced" companies , then none of us have to worry about it unless we get it back for a re-fi  or sale- but we now "know" how to deal with it.
Just keep doing what you feel comfortable with, and everything will be OK.
