Df you have a very good client. There are lenders and brokers out there who will NOT forward details from a credit report knowing it could be solved with a claim. This way, they get the deal closed. This almost happened to me twice and caught them both. 18 years in the business and still haven't seen a claim. I have no clue what one looks like. The host is correct, people make mistakes. Communication is what makes a great client. Hope you didn't loose sleep.
