This is naturally the reason that no insurer will ever insure the accuracy of any public records index anywhere ever.
  Don't just use a disclaimer, but offer explicit disclosures of these facts as well as case examples of this in each and every contract and report that you complete.  
  The lack of tracking on indexing changes, especially in this digital age, is rampant.  I doubt anyone could subpeona intelligible records of every database change from any county without the need to hire a team of computer geeks to interpret results.
