This type of fraud would be so easy to do with the knowledge we all have, but it always leaves a paper trail.   I can't imagine that anyone could get away with it, but I guess that doesn't stop people from trying.

In the situation you mentioned, the "owner" would have acquired title from the fraudulent conveyance.  It would have been a refi at that point and the company could have relied on a current owner search.  If the examiner stopped on the quit-claim deed, it would show free and clear.  Could be hard to prove fraud on his part, it could have just been a sloppy, inexperienced searcher.
