I am somewhat in agreement on your main point, actually.  The poor who get sick do have a baseline level of care under the current system-- the emergency room.  I think it's a lousy system, but everyone does have access to some level of health care.  Just like other essentials like food, clothing, and shelter, the rich get better quality product than the poor, but that actually is less signifcant than people think-- life does not necessarily require optimal conditions.  Consider the Bristlecone Pine, the longest-living form of life on earth, which lives in the poor soils on the inhospitable high ridges of the Sierra and the Great Basin, surviving harsh winter cold.... for up to 5000 years!

Conceptually it's really not that much of a stretch to compare emergency room health care to basic police and fire coverage.  For example, the rich augment their security from the baseline of police protection with any number of security augmentations-- monitored alarm systems, closed circuit cameras, gated communities with dedicated security personnel, etc.  And for the very rich, fire protection is augmented too.  And of course, the rich get their Cadillac health insurance plans or simply pay for premium Mayo Clinic care out of pocket. 

The vast majority of health care is stuff most of us can do for ourselves-- keeping active, eating a reasonable diet, not poisoning ourselves with intoxicating chemicals.  But most Americans fail to live a healthy lifestyle-- and they end up with very preventable and costly chronic disorders that drive up the cost of health insurance sky high for everyone.  Let's see a lot more Americans do the basics for themselves, and then come talk to me about moral imperatives for universal health care in America.

By the way, I think that you are slightly confused about the national police force thing.  I have seen no radical moves by Obama in this area.  We do have national law enforcement agencies-- the BATF, the DEA, the FBI, the SEC, etc, but I have not heard anything about the creation of a new national police force.  Perhaps you have seen a black helicopter that I have not! :-)
