PDFill.

I really like PDFill. You can download it to your desktop and it too is free.

Download the PDF Writer and PDF Tools. You can print anything as a pdf (writer) and the tools allow you to merger individual documents, split or reorder or delete pages, rotate pages or crop pages and convert TIFs to pdfs.

There is also a free PDF Editor which you can download and I believe can help you create forms.

Pdfill is also recommended by CNET. Hope this helps as well.
